It is important to distinguish benign variants (e.g. intermittent or slowly progressive puberty or premature thelarche) from central causes. Central causes activate the hypothalamic–pituitary gonadal axis causing secretion of gonadotrophin-releasing hormone (GnRH), increased luteinising hormone (LH) and follicular-stimulating hormone. Guidelines recommend using a basal serum LH measurement as a screening test, but many girls are still referred for expensive, time-consuming GnRH stimulation tests. Endocrinologists from Toronto assessed the reliability of LH measurement in 57 girls, mean age 6.2 ± 2.1 years, with early signs of puberty followed for a median of 5.8 months (range 3.8–10.5).1 Pubertal progression occurred in 18 girls (31.6%). All 16 girls with a basal LH level ≥0.3 IU/L had subsequent clinical pubertal progression. In contrast, 39 of 41 girls (90.5%) whose basal LH was ≤0.2 IU/L had non-progressive puberty. The endocrinologists showed that they were able to reduce GnRH stimulation tests by 75% without compromising the rate of diagnosis of central precocious puberty.
