According to the manufacturer's instructions, the SenseWear armband vibrates in a very distinctive fashion to indicate that it is ready to commence reliable data recording. In our study, this vibration was noted after 5 minutes in most patients, while it was observed after a longer time interval in a minority of patients. We, therefore, discarded the data relating to the first minutes but not regularly those relevant to the first 30 minutes. Indeed, the manufacturer does not suggest discarding the data recorded during the first 30 minutes. Version 4.0 of the software was used because this was the latest version available at the time the study was completed. Certainly, the point on new software is well taken, and we hope to evaluate the SenseWear armband using the latest version in the future. However, the producer should give detailed information on the characteristics of the new release of their software and, in particular, the reason why they would work better in extremely obese patients. Our main aim was to compare the data of indirect calorimetry with those of the SenseWear armband in obese subjects. Obviously, the SenseWear armband performed better in lean and overweight patients. However, the small number of such subjects in the study population does not impact our main conclusion that the software needs to be improved for use in obese patients, such as those enrolled in the present study. For the same reason, we believe that the preponderance of women in our study has very little effect on the endpoint. Essentially, a portable device should, by virtue of its software and hardware, be able to produce reliable measurements, irrespective of patients’ gender. Finally, there appear to be no data to support the suggestion that the SenseWear armband can be utilized for measurements of energy expenditure for weeks at a time in obese individuals. Mignault et al (2) reported excellent results compared with doubly labeled water, but this was restricted to 6 patients with a mean BMI of 30 kg/m2. Similarly, the study by Jakicic et al (3) recruited subjects with a mean BMI of 23.8 kg/m2. We agree that the SenseWear armband was shown to be highly reliable in estimating the energy expenditure at rest in lean and overweight subjects. Nonetheless, there is still a scarcity of such data for obese patients. Thus, we hope that the algorithms incorporated in the more recent versions will permit the SenseWear armband to be of value for obese subjects as well." @default.
