- W4249117175 abstract "The authors performed a randomized, prospective but unblinded comparison of periprostatic vs periprostatic and pudendal nerve block in a cohort of men undergoing transperineal prostate biopsy. They found a statistically significant but numerically small difference in pain as recorded by patients using visual analogue scales. There are 2 important questions to ask. First, how valid are the data and, second, what is the take-home message. The study seems to provide reasonably good data although there is a mixed bias resulting from the study design. It is biased toward the group not having a pudendal nerve block because the total pain score was a summation of 4 different parts of the biopsy procedure all weighted equally. Because the differences in pain scores were greatest for the biopsy itself, the real benefit would be greater if the results had been weighted according to the duration of each part of the entire procedure. In contrast, there is a bias toward the combined anesthetic group because patients knew whether they were receiving more intensive anesthetic. Regardless, these effects are probably small and we can feel comforted that the findings are valid.
