Five hundred eighty-nine term newborns were randomly allocated to receive Lactobacillus reuteri or placebo for 90 days, the primary outcome being the incidence of colic. Parents recorded duration of inconsolable crying per day. Results were overwhelmingly in favour of the probiotic group, leading some to predict that prophylactic use of probiotics may become common practice.4 We believe the study results should be interpreted with caution. No validated criteria were used to define colic, nor was there a relative risk reduction reported for the difference between treatment groups. Additionally, intention-to-treat analysis was not used, and the majority of those lost to follow up were due to unexplained protocol violations . In contrast, a well-defined study5 investigated the use of probiotics in treating established colic and reported no difference between L. reuteri and placebo groups; however, in this study, a different clinical question was addressed. With little known about the long-term safety of probiotics and studies producing contrasting results, we believe probiotics should be used with caution until there is conclusive evidence.
