- W4249254678 abstract "With the introduction of new ventilatory techniques such as intermittent mandatory ventilation and high frequency positive ventilation in the early 1970s, the mortality rate for newborn infants decreased dramatically and the incidence of complications such as pulmonary air leaks also declined. Gradually the proportion of infants surviving when born at short gestation increased even though in these groups of infants extra-alveolar air leaks, barotrauma and long-term oxygen dependency are seen more frequently. The use of antenatal steroids in the mother to improve the maturation of the fetal lung, and the introduction of neonatal surfactant therapy, have improved the survival rate for preterm infants but have not led to a corresponding decrease in the incidence of long-term oxygen dependency. Term infants with meconium aspiration and diaphragmatic hernia are other groups of infants in need of new technologies for ventilation or extracorporeal gas exchange for the prevention of long-term sequelae. In this article some of the well-established ventilatory techniques are reviewed and some new techniques are briefly presented.
