- W4249546224 abstract "日常診療で最も経験する末梢性めまいである良性発作性頭位めまい症 (BPPV) では, 特定の頭位をとると若干の潜時のあとで回転性のめまいが数十秒間起こる. BPPV に対する理学療法の有効性から, 本症の原因は卵形嚢の平衡斑より脱落して半規管内に迷入した浮遊耳石またはクプラに付着した耳石によるものと推定されている. 前者は半規管結石症, 後者はクプラ結石症と呼ばれる. 耳石の存在する半規管により後半規管型, 水平 (外側) 半規管型, 前半規管型の3つの病型に区別される. 頻度としては後半規管型 BPPV が最多であり, 水平半規管型 BPPV は10～30%, 前半規管 BPPV は2%と少ない. 診断には頭位眼振検査と頭位変換眼振検査を用いる. 頭位変化により誘発される半規管内に生ずる内リンパ流動 (もしくはクプラの偏位) に依存した眼振から病型と患側を診断する. 治療は半規管内の耳石を卵形嚢へ排出させることを目的として頭位治療が行われる. エビデンスのある頭位治療として後半規管型 BPPV の Epley 法, Semont 法が挙げられる." @default.
