In reality, hypogonadism is much more difficult to define, given the myriad of end-organ responses to circulating androgens. The standard clinical diagnosis of hypogonadism relies on a combination of laboratory measurements of androgens and an assessment of signs and symptoms related to low androgens. For example, a man may have a low-normal testosterone level but suffer symptoms suggestive of hypogonadism, such as fatigue, depression, and erectile dysfunction (ED). On the other hand, a man may have normal libido, sexual function, and energy levels but have low testosterone levels. In these scenarios, it would be difficult to assign a diagnosis of hypogonadism in the second patient without symptoms. In addition, treatment with supplemental testosterone would not be indicated. This simplified patient scenario demonstrates the difficulty in defining such a complex condition and perhaps underscores the concept of individual thresholds of testosterone actions in different target organs." @default.
