- W4251389956 abstract "Is a non-pregnant 3D transvaginal scan (TVS) necessary to detect uterine anomalies in women with recurrent pregnancy loss (RPL) who have had multiple previous 2D TVS? A retrospective review of the electronic ultrasound database of all 3D TVS performed in a specialist, tertiary referral RPL (≥3 losses) clinic. All scans were performed by the same level-2 nurse sonographer. Additional information was obtained from electronic patient records. From 2014–2020, 436 3D TVS were performed. Uterine anomalies were detected in 23/436 (5.3%). Of the 23 anomalies: 17 arcuate (73.9%), 4 sub-septate (17%), 1 bicornuate (0.04%) and 1 unicornuate (0.04%). Excluding arcuate anomalies, only the bicornuate uterus was suspected on previous 2D scanning. Of those assessed as subseptate on 3D TVS who underwent hysteroscopy, one was reclassified as arcuate and the other had resection of the confirmed septum. The patient thought to have a bicornuate uterus later had a laparoscopy for non-reproductive reasons and was diagnosed with uterine didelphys. While the prevalence of uterine anomalies is common in this sample, the most frequently identified is now considered to have the same reproductive outcomes as a structurally normal uterus. This can provide reassurance to the majority. Small numbers of inoperable but significant anomalies were identified who can be appropriately counselled about possible future pregnancy outcomes. Those who may benefit from surgical intervention can be advised. Our data suggest first trimester 2D TVS focused on assessment of the pregnancy. 3D TVS which is readily available, inexpensive and non-invasive has a valuable place in the investigation of women with RPL." @default.
