Lane1* 1 Delmarva Chiropractic PC, United States Background: An 8 year-old, healthy and active male suffered a mild traumatic brain injury (mTBI) with concussion symptoms secondary to a fall on the right lateral aspect of the frontoparietal junction. The patient suffered with a host of symptoms—the most severe including diplopia, cephalgia, choreiform movements of the cervical spine, tics and myoclonic jerks. The patient demonstrated significant decreases in his fields of gaze, aberrations of ocular function a slowing of saccadic velocity with increased latency to initiation, alterations in optokinetic induced nystagmus, and a breakdown of pursuit gain. Additional findings included decreased balance and bilateral esotropia. Methods: A treatment regimen of daily intervention was initiated for two weeks, combined with similar home protocols to be performed three to four times daily. This was gradually tapered off over the next several weeks at approximately one or two treatment per week until the patient was discharged for medical recovery after approximately ten weeks. Each session consisted of vestibular rehabilitation exercises combined with cerebellar activation and ocular motor exercises. Results: At the end of treatment there was a near normalization of all clinical findings. The patient no longer suffered with ocular dysfunction including diplopia, had a complete resolution of cephalgia, as well as the myriad of movement disorders. Additionally his ocular motor findings had improved in all parameters. Conclusions: Functional neurological multi-modal applications including ocular motor, vestibular and other therapeutic interventions may be viable treatment methods to improve neurological function and assist in the recovery of movement disorders and other cortical phenomena secondary to TBI and concussion. The author further suggests investigation and application of multi-modal and functional neurological applications into case presentations of this type.
