- W4280582364 abstract "Over the past two decades, we have seen a steady increase in demands for the anti-nuclear antibody (ANA) test. The reasons are attributable to a growing interest in the early diagnosis of systemic autoimmune diseases also by general practitioners and non-rheumatological specialists, the tendency to consider ANA research as a screening rather than as a confirmatory test and, finally, the spread in clinical laboratories of new more standardized automatic analytical methods characterized by high diagnostic specificity. In this scenario, the ANA test, although present in the clinical laboratory repertoire for over 50 years, continues to be the subject of controversy both as regards the optimal detection method and the interpretation of the result and its clinical use. The indirect immunofluorescence method on HEp-2 cell substrates (IFI HEp-2) is historically the most used for ANA research but is affected by a series of critical issues that have favored the development of new solid phase immunometric analytical procedures intended for complement and integrate the IFI HEp-2 test in order to achieve greater diagnostic accuracy. Finally, the advent of multiparametric analytical procedures has opened up new perspectives in diagnosis, therapy monitoring and outcome prediction even in systemic autoimmune diseases. In this review, we consider the role of ANA in screening for systemic autoimmune diseases in the light of new analytical technologies, the paradigm shift in pre-test probability assessment and correct interpretation of results." @default.
