- W4280604920 abstract "Patients with symptomatic and asymptomatic carotid stenosis are faced with multiple treatment options, including best medical therapy, carotid endarterectomy, transfemoral carotid stenting, and, more recently, transcarotid arterial revascularization (TCAR). In contemporary practice, an emphasis on shared decision making has been shown to increase patient satisfaction and adherence to therapies. As part of this process, decision aids can be useful to present each option to patients in a standardized fashion to assist patients in making a decision which is consistent with their healthcare goals and values. In this review, we sought to identify current decision aids available for carotid stenosis and determine whether external decision aids represent a useful adjunct to shared decision making for patients facing treatment decisions. A literature review of the Cochrane database and PubMed of terms, including patient preference, decision aid, and shared decision making, combined with carotid stenosis and carotid artery disease was conducted. The abstracts of the resulting studies were reviewed, and the studies considered relevant were read in their entirety. Studies that had evaluated the use of a decision aid for patient decision making in carotid disease were included. A total of 475 studies resulted from the search. After review, we identified four studies that had evaluated decision aids for carotid stenosis and two with proposed protocols for evaluating decision aids in future studies. None of the decision aids included information regarding TCAR. Three of the four studies had used standardized patients who did not have carotid disease. In studies asking patients to choose between medical management and intervention, approximately one half of the patients had chosen intervention after viewing the decision aids. In studies considering the format of the visual aids, the patients had preferred bar graphs and pie charts over icon arrays. Qualitative presentation of the information compared with the absolute risk of adverse events predicted for the patients choosing surgery over medical management. In multiple studies, the participants choosing surgical intervention tended to be younger and male. This review of the current literature revealed a gap in the standardized use of decision aids for patients with carotid stenosis, especially with the advent of TCAR as a treatment option. Further development, evaluation, and use of decision aids could help patients with carotid stenosis make more informed therapeutic decisions consistent with their goals and values." @default.
