The available evidence supporting the causal relationship between nonobstetric surgery and teratogenicity, preterm labor/delivery, or early pregnancy loss remains questionable; therefore withholding indicated surgery during pregnancy is not recommended by either the American Society of Anesthesiologists (ASA) or the American College of Obstetricians and Gynecologists (ACOG). Nevertheless, until better studies are performed, elective surgery is not encouraged during pregnancy. Surgery or the use of general anesthesia during pregnancy should occur only out of necessity. Reassuring the mother while educating her about the safety of anesthesia during pregnancy is important. When indicated, anesthesia for nonobstetric surgery can be administered safely. The decision to use fetal monitoring during surgery should be individualized using a multidisciplinary team-based approach per ASA and ACOG guidelines. In essence, the clinical relevance of animal findings addressing anesthetic neurotoxicity and widespread apoptosis in fetal brains is unclear, and more studies are needed to translate these findings into human clinical practice.
