Провести комплексный анализ анамнестических данных, включающий основные экстрагенитальные и гинекологические заболевания, репродуктивный анамнез, оценить объективный статус пациенток, вклад клинико-анамнестических данных в прогнозирование имплантации при проведении циклов вспомогательных репродуктивных технологий (ВРТ).Материалы и методы. В проспективное исследование была включена 121 пациентка, находящаяся в программе ВРТ отделения планирования семьи и вспомогательных репродуктивных технологий государственного учреждения «Республиканский научно-практический центр «Мать и дитя» (РНПЦ «Мать и дитя») с января 2019 г. по июнь 2020 г. По результатам проведенных программ экстракорпорального оплодотворения (ЭКО) женщины были разделены на две группы: первую – основную группу (ОГ) – составили 60 пациенток, у которых клиническая беременность наступила, вторую – группу сравнения (ГС) – составила 61 пациентка, у кого клиническая беременность не наступила. Проведена сравнительная ретроспективная оценка объективного статуса, соматического, гинекологического и репродуктивного анамнеза у пациенток с бесплодием, находящихся в программе ВРТ, в зависимости от наступления имплантации.Результаты. В результате проведения оценки клинико-анамнестических данных достоверно установлено, что имплантация наступала реже у пациенток позднего репродуктивного возраста с нормальным уровнем антимюллерова гормона (АМГ) (p=0,002). Фактором объективного статуса, определяющим исходы ВРТ, явилось наличие избыточной массы тела (р=0,019). При анализе гинекологической патологии в группе с неблагоприятными исходами достоверно чаще диагностировались воспалительные заболевания малого таза (р=0,005), миома матки (р<0,001), все формы наружного генитального эндометриоза – поверхностные формы эндометриоза (р<0,001), глубокий инфильтративный эндометриоз (р=0,021). Также пациентки данной группы достоверно чаще в анамнезе имели полипы эндометрия (р=0,006).Выводы. Изучение клинико-анамнестических данных является необходимым этапом обследования пациенток в циклах ЭКО. Полученные данные могут быть использованы для предварительной оценки результатов программ ВРТ на этапе сбора анамнеза и оценки объективного статуса пациентки. На основании проведенного сравнительного анализа на стадии описательной статистики произведен отбор клинико-анамнестических показателей, ассоциированных с имплантацией эмбриона в программах ВРТ, для дальнейшего многофакторного анализа и построения математико-статистической прогностической модели с целью возможности прогнозирования исходов циклов ЭКО. Purpose. Conduct a comprehensive analysis of anamnestic data, including the main extragenital and gynecological diseases, reproductive history, assess the objective status of patients, evaluate the contribution of clinical and anamnestic data to the prediction of implantation during ART cycles.Materials and methods. The prospective study included 121 patients who were in the ART program of the Department of Family Planning and Assisted Reproductive Technologies of the State Institution Republican Scientific and Practical Center Mother and Child from January 2019 to June 2020. According to the results of IVF programs, women were divided into two groups: the first group – the main group – consisted of 60 patients in whom clinical pregnancy occurred, the second group – the comparison group – consisted of 61 patients in whom clinical pregnancy did not occur. A comparative retrospective assessment of the objective status, somatic, gynecological and reproductive history in patients with infertility, who are in the program of assisted reproductive technologies, depending on the onset of implantation, was carried out.Results. As a result of the evaluation of clinical and anamnestic data, it was reliably established that implantation occurred less frequently in patients of late reproductive age with normal levels of anti-muller hormone. The objective status factor determining the outcomes of ART was the presence of overweight. When analyzing gynecological pathology in the group with unfavorable outcomes, pelvic inflammatory diseases, uterine fibroids, all forms of external genital endometriosis – superficial forms of endometriosis, deep infiltrative endometriosis were significantly more often diagnosed. Also, patients of this group were significantly more likely to have endometrial polyps in the anamnesis.Conclusions. The study of clinical and anamnestic data is a necessary stage of examination of patients in IVF cycles, the data obtained can be used for preliminary evaluation of the results of ART programs at the stage of collecting anamnesis and assessing the objective status of the patient. Based on the comparative analysis carried out at the stage of descriptive statistics, the selection of clinical and anamnestic indicators associated with embryo implantation in ART programs was made for further multifactorial analysis and the construction of a mathematical and statistical prognostic model in order to be able to predict the outcomes of IVF cycles." @default.
