- W4282920662 abstract "To the Editor: Previous studies have used B-cell lymphoma 6 (BCL6) testing during unstimulated ovulatory cycles to prognosticate endometrial receptivity during fresh in vitro fertilization (IVF) stimulation cycles ( 1 Klimczak A.M. Herlihy N.S. Scott C.S. Hanson B.M. Kim J.G. Titus S. et al. B-cell lymphoma 6 expression is not associated with live birth in a normal responder in vitro fertilization population. Fertil Steril. 2022; 117: 351-358 Abstract Full Text Full Text PDF PubMed Scopus (3) Google Scholar ). In our study, endometrial biopsy samples were collected from women undergoing ovulatory stimulation through assisted reproductive techniques during the midsecretory phase (8 days after trigger injection), which would be more similar to receptivity during a fresh embryo transfer cycle ( 2 Almquist L.D. Likes C.E. Stone B. Brown K.R. Savaris R. Forstein D.A. et al. Endometrial BCL6 testing for the prediction of in vitro fertilization outcomes: a cohort study. Fertil Steril. 2017; 108: 1063-1069 Abstract Full Text Full Text PDF PubMed Scopus (27) Google Scholar ). We argue that performing a biopsy during an unstimulated ovulatory cycle and then using it to predict a fresh embryo transfer is illogical. To clarify, our biopsies were not performed during the frozen embryo transfer cycle as specified in point 2; therefore, concerns regarding its effects on implantation are mute. It is unlikely that an oocyte retrieval, which does not involve the uterus, would disrupt the endometrium and result in altered BCL6 expression. We did not use the ReceptivaDx test in this study. B-cell lymphoma 6 (BCL6) testing before in vitro fertilization as a predictor of failureFertility and SterilityVol. 118Issue 2PreviewWe read with great interest the article published by Klimczak et al. (1), and we have several concerns. First, the investigators tried to compare results with our previous work in which we performed biopsies in ovulatory cycles before in vitro fertilization stimulation (2). In this study, Klimczak et al. (1) examined samples in ovarian hyperstimulated cycles. The timing of performing a biopsy is important because serum progesterone levels are altered during hyperstimulation, a biopsy performed close to the time of embryo transfer can potentially affect implantation, and retrieval itself can cause inflammation and potentially influence B-cell lymphoma 6 (BCL6) expression.
