- W4283156695 abstract "TYPE: Abstract TOPIC: Disorders of the Pleura PURPOSE: Lung entrapment is common in patients with malignant pleural effusion (MPE). These patients are best palliated with chronic indwelling pleural catheters or intermittent drainage instead of pleurodesis. The recognition of predictors of lung entrapment may be important due to the implications of its therapeutic approach. METHODS: Patients diagnosed with MPE between January 2012 and December 2018 in our hospital center were retrospectively analyzed. Demographic and clinical data of patients were reviewed. The group of patients with and without trapped lung (TL) were compared. A univariate and multivariate binary logistic regression analysis were used to determine predictors of the development of TL. RESULTS: A total of 365 patients with MPE (52.1% were male, mean age of 68.0±12.5 years) were analyzed, of who 68 (18.6%) had TL. In univariate analysis, lung-cancer (p=0.004), loculated effusion (p≤0.001), left MPE (p=0.034), bilateral MPE (p=0.004), most recurrent drainages (p=0.001), higher pleural fluid protein (p=0.034) and higher effusion-LDH/serum-LDH ratio (p=0.021) were significantly associated with TL. In the multivariate analysis, lung-cancer (OR:2.47; p=0.025), loculated MPE (OR:7.06; p≤0.001), recurrent drainages (OR:1.22; p=0.019) and effusion-LDH/serum-LDH ratio (OR:1.10; p=0.026) remained significantly associated with TL. No significant differences regarding age, sex, ECOG-PS, effusion volume at diagnosis, time between cancer and MPE diagnosis and effusion macroscopic characteristics were found between the groups. CONCLUSIONS: In our analysis, lung cancer, loculated MPE, and a higher effusion-LDH/serum-LDH ratio were the independent predictors found for TL. CLINICAL IMPLICATIONS: This suggests the need for early pleurodesis in lung-cancer patients with MPE and a higher effusion-LDH/serum-LDH ratio. DISCLOSURE: Nothing to declare.
