- W4283156737 abstract "TYPE: Abstract TOPIC: Obstructive Lung Diseases PURPOSE: To evaluate the influence of the ventilation mechanical (MV) modes on the total and regional lung ventilation and aeration of critically ill obstructive patients. METHODS: In this blind clinical trial, the eligible patients underwent nebulization with salbutamol sulfate and ipratropium bromide in the ventilation mode they were found: controlled, assisted-controlled or spontaneous ventilation modes. The evaluation of electrical impedance tomography was performed before and after the intervention and the variables analyzed were related to ventilation and pulmonary aeration and the regional lung distribution by impedance variation in the region of interest. Pulmonary mechanics data were also collected. A joint and stratified analysis into ventilation modes groups were done, being p<0.05. RESULTS: From 19 procedures, 5 occurred in controlled MV mode, 7 in assisted and 7 in spontaneous. In intragroup analysis the nebulization increased total ventilation in controlled (p= 0.04, ⅆ= 2) and spontaneous (p=0.01 and ⅆ=1.5) MV modes. The ROI analysis showed that there was an increase only in ventilation in dependent pulmonary region in assisted mode (p=0.01 and ⅆ=0.3) and in spontaneous mode (p=0.02 and ⅆ=1.6). There was no difference in intergroup analysis. CONCLUSIONS: There was an increase on total ventilation in controlled and spontaneous MV modes after nebulization. The regional analysis shows differences in lung mechanics responses after nebulization procedures, with increase of peak expiratory flow and reduction of airway resistance. CLINICAL IMPLICATIONS: The effectiveness of MV inhalation therapy may be influenced by different ventilatory modes.
