- W4283162855 abstract "TYPE: Late Breaking Abstract TOPIC: Allergy and Airway PURPOSE: 1.To assess the etiology of bronchiectasis by utilising virtual and fibreoptic bronchoscopy. METHODS: Tracheopathia osteochondroplastica is a rare airway disorder,where there is benign dysplasia of the trachea and large bronchi, characterized by calcifying cartilaginous excrescences into the tracheal lumen. Most of the cases reported were diagnosed as incidental findings .Bronchoscopic findings of TO ranges from 0.01% -0.80%.The prognosis of this condition is usually excellent, with airway obstruction progressing slowly. Intervention is indicated only if there are symptoms of airway stenosis. Treatment is symptomatic including antibiotics, mucolytics and hydration. RESULTS: Our patient presented with acute exacerbation of bilateral bronchiectasis with complaints of shortness of breath and wheeze. CT Thorax with virtual bronchoscopy showed hyperdense irregular nodular thickening of anterior and lateral walls of lower trachea with cylindrical bronchiectasis(FIG 1).Sputum AFB was negative.Bronchoscopy was done which showed multiple beaded cobble stone appearance in anterior and lateral walls of trachea and bronchi (Moderate stage) and Histopathological examination showed cartilaginous and osseous submucosal nodules consistent with diagnosis of TO.She was managed with inhaled steroids,antibiotics and mucolytics and is on regular follow up. CONCLUSIONS: TO is a rare diagnosis made on basis of CT scan,bronchoscopic findings and histopathological examination.Bronchoscopic differential diagnosis of TO with other diseases such as amyloidsis,papillomatosis,sarcoidosis and polychondritis based on sparing of posterior wall of trachea and histopathological examination is necessary. CLINICAL IMPLICATIONS: The prognosis of TO is favourable with acute exacerbations managed with inhaled steroids.Hence patients with acute exacerbations of bronchiectasis should be evaluated for TO to prevent further complications including tracheal stenosis.
