- W4283164870 abstract "<h3>Introduction</h3> In Leicestershire, a pathway has been established from primary care for patients > 40 years old with weight loss and/or abdominal pain, or palpable abdominal mass – in which a GP can request a straight to test (STT) CT scan of abdomen & pelvis with contrast to assess for a gross/disseminated abdominal cancer. There are concerns whether this pathway has been misused and that patients who should have undergone and colonoscopy or CT colonogram for iron deficiency anaemia (IDA) and/or change in bowel habit (CIBH) may have had colorectal cancer (CRC) missed due to unprepared CT missing small mucosal masses, or advanced colonic polyps, the administration of this pathway is by non-clinical staff. We aim to evaluate this pathway. <h3>Methods</h3> The CRIS radiology software was interrogated for the first 200 referrals from January 2020 to this STT CT pathway. GP referral data including blood test results were analysed. Subsequent scans were reviewed in a 2 year follow up period. <h3>Results</h3> <b>200 patient</b><b> scan results reviewed, 157 were misreferred (78.5%) -</b> see table 1. Mean/median age of patients: 68.7/69.0 years (range 41 - 99). Number of scans in which pathology was reported = 31 (15.5%) - 29 cancers (14.5%) Pancreatic cancer = 9,CRC = 8, Lung cancer = 4, Gynaecological cancer = 2 Prostate cancer = 2, Other cancers = 4 (oesophagus, HCC, cholangiocarcinoma, gallbladder) Benign disease = 2 Of the 169 with a normal CT scan reported – none of these patients had follow up scans in the following two years demonstrating a cancerous lesion. <h3>Discussion</h3> Analysis of 200 patients shows a very high yield of cancer (14.5%). The concerns regarding the misuse of this pathway remain as the majority didn’t meet the specific criteria. Reassuringly, none of the patients with normal CT scans had scans showing a cancerous lesion in two year follow up. This STT pathway is useful for GPs who have concerns of cancer – including GI cancers." @default.
