- W4283168021 abstract "TYPE: Abstract TOPIC: Chest Infections PURPOSE: Inflammatory markers are often elevated in SARS-CoV-2 pneumonia, including ESR, CRP, ferritin, D-Dimer. Objective of this study is to assess prognostic ability of inflammatory markers, and any correlation with mortality. METHODS: We conducted retrospective analysis in local hospital in California. Patient population included 250 patients, adults above 18 years, tested positive via nasopharyngeal RT-PCR assays between 03,2020 to 06,2020. Inflammatory markers were trended every three days. RESULTS: Our study showed variation in inflammatory biomarkers without correlation between biomarkers values and hospital discharge vs death as suggested by overlapping 95% confidence bands. At no point did CRP, ferritin, D-Dimer; regardless of collection of day 1, 3, or 5 meet statistical significance (P <0.05). The only statistically significant results were CRP average on day 5 which was 30.1 (CI: 22.6-40.2) and 63.5 (CI: 50.2-80.2) (P<0.05). With regards to outliers, their inclusion or lack of inclusion failed to meet significant statistical significance. Local polynomial regression method is robust to outliers. Limitations for this study included smaller sample size and skewness in distribution. Because of skewness in distribution, time series plots of all patients are averaged over time using local polynomial regression based on lab values in the log scale. CONCLUSIONS: Inflammatory markers were abnormal in majority of patients, however no significant prognostic association could be drawn between elevated inflammatory markers and adverse outcomes. Discontinuing checking markers may help reduce cost of healthcare as well. CLINICAL IMPLICATIONS: Inflammatory markers may indicate ongoing clinical process, clinical increment should be used as primary guide for management of patients with SARS-CoV-2 pneumonia.
