- W4283168528 abstract "<h3>Aims</h3> The SMSA (size morphology site access) scoring system is a commonly used tool by colonoscopists to grade colonic polyps. The SMSA scoring may be subjective and may have variation between different clinicians. We evaluated the scoring system to see if there’s any variation between colonoscopists when assigning a score to a given polyp. This allows us to understand the effectiveness of the tool and look for areas where it can be improved <h3>Methods</h3> Ten videos containing polyp each were sent using JotForm website as a survey to 100 colonoscopists. The participants included members of Yorkshire endoscopy network, BSG endoscopy committee members and BGEQIP regional leads. <h3>Results</h3> Agreement on individual parameters of the scoring system were analysed for 10 different polyps (size, morphology and access) along with the overall agreement when assigning a polyp SMSA level. The size parameter showed the most variation and gave an agreement level of kappa 0.241 (fair agreement), which was the parameter with lowest score for agreement. The access and morphology parameter give us an agreement value of 0.41 and 0.505 respectively, both showing moderate agreement. The overall agreement for SMSA level was kappa 0.346 which gives us fair agreement. The overall agreement for the SMSA level of the experienced group (colonoscopists with >1000 lifetime colonoscopies) was 0.361. Access was the only parameter that had a lower agreement when looking at the experienced group (0.398) <h3>Conclusions</h3> There is a considerable variation between the scores assigned by different clinicians and these variations are most pronounced in the size parameter of the score. This suggests that polyps’ size is the parameter that has the largest influence in the variation of overall SMSA level." @default.
