- W4283171712 abstract "<h3>Introduction</h3> Development of ascites is an important milestone in the natural history of cirrhosis; 20% of patients presenting with ascites die in the first year. Large volume paracentesis (LVP) has been the established treatment for symptomatic refractory ascites, but caries 1% risk of significant complication. However, there are no devices designed specifically for the purpose of paracentesis. We hypothesized that a bespoke ‘Melody’ catheter designed to improve the flow of ascitic fluid would minimise incomplete drainage and prolong interval between LVP hence reducing complications. We aim to compare the performance of the ‘Melody’ device with the currently used catheter for LVP. <h3>Methods</h3> In this feasibility randomised controlled trial, we prospectively recruited patients undergoing LVP at the Queens Day Case Unit, Nottingham University Hospitals. During each LVP session, patients were randomised to either standard catheter, ‘Melody’ device or ‘Melody’ device with an additional pump. The volume of ascites drained and time taken for the paracentesis were evaluated. Adverse events including fluid leak, pain requiring analgesia and bleeding were recorded. <h3>Results</h3> 58 patients who attended for 99 LVP sessions were randomised; 40 paracentesis were carried out with the standard catheter, 39 with ‘Melody’ drain and 20 with ‘Melody’ drain attached to the pump. There was no difference in the volume of ascites drained per session and the rate of drainage between the 3 groups (Table 1). There were more adverse events reported with the ‘Melody’ drains, and this was related to ascites leakage and pain (Table 1). <h3>Conclusions</h3> The customised ‘Melody’ drain performed as well as the conventional catheter but with more reported adverse events; particularly pain, incision related bleeding and post-procedure ascitic leakage related to the larger drain calibre. The addition of pump did not improve the rate of ascites drainage. This RCT also demonstrate the feasibility of patient recruitment via the day case setting." @default.
