- W4283525818 abstract "Drug-related problem (DRP) is a concerning issue in all healthcare systems. Pharmacy-based interventions play a vital role in preventing and reducing their occurrence. In Middle Eastern countries, involving Qatar, the professional roles of pharmacists have been expanding over the past few years as a strategy to improve patient safety. The purpose of this study was to characterize and analyze the pharmacist documented interventions among hospitalized patients in a general hospital in the state of Qatar. A cross-sectional retrospective analysis of electronically reported clinical pharmacist interventions occurred among hospitalized patients admitted to Hamad General Hospital in Qatar was used. The data was extracted from a period of three months (from March 1, 2018 to March 31, 2018, from July 15, 2018 to August 15, 2018, and from January 1, 2019 to January 31, 2019) and consisted of patients’ and interventions’ characteristics. Continuous variables were presented as means (± standard deviation (SD)), whilst frequencies and percentages were used for categorical variables. A total of 340 patients with 858 interventions were included in this study. The average age of the study participants was 51 years (SD± 17.66). The study population was predominantly male (n=280, 82%). The prevailing pharmacist interventions were addition of a drug therapy (27%) followed by medication discontinuation (18%) and dosage adjustments (16%). This pattern was maintained across all subcategories (e.g, gender, age, ward) except for emergency department, where cessation of a medication was found the most (n=37). The two pharmacological class categories associated with the majority of interventions were anti-infective and cardiovascular agents. Clinical pharmacist interventions in a general hospital in Qatar are effective in identifying, preventing, and resolving DRPs in inpatient settings. Future studies should consider prospective design and incorporate a content quality assessment of interventions." @default.
