- W4285113819 abstract "People with spinal cord injury (SCI) experience a dramatic and early muscle wasting below the level of the lesion which results in a substantial decrease in overall daily energy expenditure and, hence, fat mass increase. These changes in body composition underlie the pathophysiology of metabolic syndrome (MetS), a cluster of visceral obesity, insulin resistance, hypertension, and dyslipidemia. Diagnostic criteria for MetS, largely accepted in the able-bodied general population, result to be inappropriate in SCI. For the same value of body mass index (BMI), people with SCI exhibit higher percentages of body fat than able-bodied individuals; therefore, the use of traditional cut-off points of BMI grossly underestimates the diagnosis of overweight and obesity. On the contrary, the measure of waist circumference (WC) could overestimate visceral obesity due to abdominal muscle paralysis. Even diagnosis of hypertension can be challenged by the SCI-related neurogenic derangement of the blood pressure control. Although traditional models for the assessment of cardiovascular risk are largely unsuitable in people with SCI, MetS largely accounts for the unfavorable cardiovascular and cardiometabolic profile in this population. To complete the picture of such a deranged metabolic framework, among the correlates of MetS, androgen deficiency represents a very high prevalent concern in males with chronic SCI, where low testosterone displays complex and multi-directional pathogenetic links with visceral obesity. However, it is a challenge to establish whether the biochemical androgen deficiency can result in clinical hypogonadism as, following SCI, clinical features suggestive of hypogonadism could represent a reflection of neurological damage and disability." @default.
