- W4285170014 abstract "Polycystic ovarian syndrome (PCOS) is a complex endocrine disorder affecting up to 10% of reproductive-age women. PCOS is characterized by three hallmark features: hyperandrogenism, oligo-/anovulation, and polycystic ovaries. Insulin resistance is also prevalent in the majority of women with PCOS, both in the lean and obese phenotypes. Other clinical manifestations of PCOS include an increased risk of obesity, type II diabetes, and cardiovascular disease. In addition to hyperandrogenism and insulin resistance, chronic inflammation is another important contributor to the pathophysiology of PCOS. The compounding effects of these features culminate in reproductive dysfunction. Due to ovarian and uterine abnormalities, women with PCOS suffer from infertility and diminished ability to carry a pregnancy to term. Ovarian aberrations include abnormal follicular development, oligoovulation/anovulation, and reduced egg quality. Uterine abnormalities include altered endometrial gene expression, increased inflammation, and decreased endometrial receptivity to implantation. This chapter reviews the mechanistic effects of hyperandrogenism, insulin resistance, and inflammation on the ovaries, uterus, and placenta, leading to reproductive dysfunction through reduced ability to ovulate, decreased endometrial receptivity, implantation failure, placental dysfunction, recurrent pregnancy loss, and various pregnancy complications including preterm birth and preeclampsia." @default.
