Over the years, we have learned that fracture healing is determined by a complex interplay between biology and mechanics. So the focus has now shifted from fixing everything with absolute stability to providing optimal stability while causing the least possible damage to the local biology. The simple fracture patterns are preferably treated with compression and absolute stability to achieve a primary union without callus formation. Interfragmentary compression is best achieved with lag screws and plating techniques. Multifragmentary fracture patterns are preferably treated with relative stability to achieve secondary union with callus formation. It is essential to keep the strain within a band – with which induces callus formation but does not reach a critical tolerance level beyond which the callus fails to form. Bridge plating, intramedullary nails, and external fixators often provide relative stability. Meticulous soft-tissue handling and indirect reduction technique and where possible, a minimally invasive approach should be used to preserve local biology. Newer implant designs which, have a minimal bony footprint and allow locking screw fixation further prevent damage to the local blood supply. Most implants can provide a spectrum of stability and the surgeon must choose from a wide array of implants to achieve a biomechanical environment which best suits the persona of the fracture." @default.
