- W4285308423 abstract "Nosocomial infections are a major cause of morbidity and mortality in patients with traumatic brain injury. They are mainly represented by pneumonia acquired under mechanical ventilation. This susceptibility to nosocomial infections is a multifactorial phenomenon. The systemic inflammatory response syndrome (SIRS) following the trauma is a physiological phenomenon that leads to a compensatory anti-inflammatory response (CARS) in order to allow a return to homeostasis. This CARS involves the sympathetic, parasympathetic, and hypothalamic-pituitary systems. These three systems maintain an anti-inflammatory response in close connection with the cells of innate immunity via neurohormonal mechanisms. TBI also induces dysbiosis involving the intestinal and pulmonary microbiota. An impairment of the hypothalamic-pituitary–adrenal axis is also observed called “post-traumatic critical illness-related corticosteroid insufficiency.” When all these compensatory responses are exacerbated and prolonged over time, it induces a state of immunosuppression that favors the occurrence of nosocomial infections." @default.
