Более 70 пациентов отмечают влияние на профессиональную деятельность и снижение способности к обучению и около 90 отмечают влияние на качество жизни. В основе АР лежит IgE-зависимая реакция слизистой оболочки носа на стимуляцию ее аллергенами при непосредственном контакте. Первая фаза заболевания характеризуется немедленным IgE опосредованным ответом тучных клеток, проявляется гистамин-опосредованными эффектами вторая, поздняя, фаза постоянным аллергическим воспалением. Понимание основных патофизиологических механизмов аллергического ринита является залогом эффективности терапии. Исключение контакта с аллергенами играет роль этиотропной терапии и рекомендуется для всех без исключения пациентов. Сложная взаимосвязь указанных механизмов обуславливает многоступенчатый алгоритм патогенетической терапии АР, направленной на ключевые звенья патогенеза воспалительной реакции на соответствующей фазе иммунного ответа. На ранней стадии заболевания первой линией терапии являются препараты, нивелирующие эффект выделения избыточного количества гистамина и соответствующие патологические эффекты на органы-мишени. К таким препаратам, прежде всего, относятся антигистаминные препараты, связывающие рецепторы гистамина на конкурентных принципах. На поздней стадии возникает необходимость в комплексных препаратах, с воздействием на несколько патогенетических звеньев иммунного ответа, активирующих аллергическое воспаление. Препаратами выбора здесь являются интраназальные кортикостероиды. В случае отсутствия клинического эффекта от комбинированной терапии назначается терапия антагонистами лейкотриеновых рецепторов, а также рассматривают ряд других дополнительных препаратов. Отдельное место в фармакотерапии занимает специфическая иммунотерапия. Хирургическое вмешательство показано только определенной категории пациентов с АР, а основной целью такого лечения является восстановление нормального носового дыхания. В статье рассмотрены ключевые моменты применения различных групп препаратов, используемых в алгоритме терапии АР в соответствии с международными рекомендациями. Allergic rhinitis (AR) is an intermitting or persisting inflammation of upper airway mucosa that accompanied with following typical symptoms: nasal congestion and rhinorrhea, pruritus, sneezing. Sometimes symptoms are combined. More than 70 of patients report the AR impact on professional activity and decrease of learning ability and about 90 of patients report the impact on quality of life. AR is based on Ig-E-dependent reaction of mucosa to direct contact stimulation by allergens. The first phase of the disease is characterized by immediate Ig-E-mediated response of mast cells that manifests in histamine-mediated effects. The second, late phase is characterized by persistent allergic inflammation. The understanding of main AR pathophysiological mechanisms is key to the effectiveness of therapy. Avoidance of exposure to allergen is an etiotropic therapy and recommended for all patients without exceptions. Complex interactions of above mentioned mechanisms define multilevel algorithm of pathogenetic AR therapy and targeting the key points of inflammatory pathway accordingly to phase of immune response. The first line therapy of the early phase of disease are drugs that mitigate excessive histamine release and consequent damaging effects on targeted organs. These drugs first of all include antihistamines that bind to histamine receptors on concurrent basis. The late phase needs complex therapies affecting multiply pathogenetic points involved in activation of allergic inflammation. At this stage intranasal corticosteroids are the drugs of choice. In case of poor clinical response to combination therapy antagonists of leukotriene receptors should be administered and other additional therapies should be considered. Specific immunotherapy possesses a distinct position in AR treatment. Surgical intervention is indicated for only specific group of patients with AR and the main purpose is restoration of nasal breathing.
