- W4289668401 abstract "The optimal approach to risk-stratification and management of anomalous origin of the coronary arteries (AOCA) remains controversial This study aims to describe the prevalence and characteristics of AOCA in individuals undergoing computed tomography coronary angiography (CTCA) and evaluate real-world management and outcomes. A retrospective analysis was performed on all 64-slice multi-detector CTCA scans conducted at an adult Australian tertiary hospital since February 2015. Individuals with AOCA were identified and data was obtained from electronic medical records. Of 3,224 CTCA scans in 3,162 individuals, 38 (1.2%) had AOCA with median age 54 years (IQR 44-66) and two-thirds were male. Anomalous right coronary artery (RCA) was most common (n=20, 52.6%), followed by the circumflex artery (n=10, 26.3%). An interarterial course was seen in 20 individuals (52.6%, with 90% RCA), intramural course in four (10.5%, all RCA), and “slit-like” ostium in nine (23.7%, all RCA). Functional testing was performed in 14 individuals (36.8%) and four (28.6%) demonstrated provocable ischaemia. Four individuals (10.5%) underwent coronary artery re-implantation: two anomalous RCA with interarterial course and “slit-like” ostium; one anomalous left coronary artery diagnosed after aborted cardiac arrest; and one left coronary artery from pulmonary artery. The prevalence of AOCA in this contemporary study of adults undergoing CTCA is 1.2% and use of functional testing was variable. Notably, anomalous RCA was most common, and the majority did not undergo surgery despite demonstrating a malignant course or “slit-like” ostium. More research is needed to inform risk-stratification and management, especially for anomalous RCA." @default.
