- W4292121010 abstract "<h3>Aims</h3> Young People (YP) age 16-17 are presumed in UK law, like adults, to have the capacity to consent to treatment. Department of Health recommends that it is good practice to encourage 16-17 yr olds to involve their families in decisions about their care, unless it would not be in the YP’s best interests to do so, but allowing parents to decide about treatment without the YP being the final arbiter systematically disempowers YP. Our trust admits all patients up to the age of 18 to the paediatric ward. We noticed that consent form 2, ‘Parental agreement to investigation, treatment or procedure for a child or young person ‘ was frequently being used for 16-17 year olds, with a parent/guardian being the primary signatory; instead of patients signing for themselves on consent form 1 ‘Patient agreement to investigation, treatment or procedure’ . We aimed to raise awareness among surgeons and empower YP to rightfully sign their own consent forms. <h3>Methods</h3> All patients age 16-17 (excluding those lacking mental capacity, who were separately studied), admitted for procedures under general anaesthesia in the relevant time periods were included in the analysis. Data was collected retrospectively from medical notes and consent forms using an audit proforma. Information collected included, surgical consultant and speciality as well as consent form used. There was an initial 4 month data collection. Information on all specialities was circulated to all consultant surgeons along with a reminder of the legal rights of this age group. A flow chart was developed indicating which consent form to use. This was available in consent clinics and on the paediatric ward. Paediatric nurses were also encouraged to place form 1 in the medical notes for appropriate patients. Four subsequent two monthly re-audits were performed. After each re-audit, all results were emailed to surgical consultants with traffic light colour coding for each speciality. • <b>Red</b> 50% or less using form 1 • <b>Green</b> 75% or more using form 1 • <b>Amber</b> between these levels <h3>Results</h3> Results were analysed overall and also according to the 4 main surgical specialties operating in our trust. An ‘others’ category is shown for specialities with infrequent procedures in this age group. (figure 1). Our initial study showed that 41/78 , 53% were correctly consented on form 1. Speciality results varied from 19/27, 70% in speciality A to 0/6 in speciality D. Following our interventions, the overall percentage showed a steady increase up to 84% by the fourth re-audit, by which time all surgical specialities showed consent levels on form 1 above 75%. (<b>green</b>) <h3>Conclusion</h3> This simple process of audit and regular feedback to surgical colleagues has enabled YP to exercise their legal right to consent for their own treatment. Paediatricians are aware that children are not just small adults when advocating for their care . It is also important to appreciate the different rights of YP at the upper end of the paediatric age range to ensure that they are not infantilised and are empowered to appropriately participate in their own management." @default.
