It was first described by Jaensch in 1928 after orbital trauma in his case and has the same motility pattern as congenital Brown's syndrome. For this reason, in 1973 Brown differentiated between the true and simulated cases. Further clinical findings of the different etiological factors must be considered in order to differentiate between the two groups.The cause is an acquired restriction of the free passage of the superior oblique tendon through the trochlea. In most cases this is produced by a palpable swelling/nodule of the superior oblique tendon posterior to the trochlea. There are three possibilities to develop a swelling/nodule: 1. Shortly after birth due to an incomplete development at the time of birth of the sliding factors needed for a free passage. 2. An inflammation in combination with a systemic disease, such as rheumatism or idiopathic. 3. A blunt orbital trauma causing a hematoma of the superior oblique tendon. Additionally, the trochlear passage can be narrowed by a severe inflammation involving the trochlea, which is associated with a swelling and marked tenderness of the trochlear area and corresponds to stenosing tenosynovitis of the hand.The therapeutic management of these four variations differs significantly depending on the cause of the swelling.The swelling of the superior oblique tendon posterior to the trochlea explains the motility disorder in acquired Jaensch-Brown syndrome. There are three different causes for the swelling, which require different therapeutic management.KRANKHEITSBILD: Beim erworbenen Jaensch-Brown-Syndrom handelt es sich um eine erstmals von Jaensch 1928 beschriebene, bei seinem Fall nach einem Orbitatrauma aufgetretene mechanische Einschränkung der Hebung in Adduktion mit Parallelstand im Abblick. Dies entspricht der beim kongenitalen Brown-Syndrom vorliegenden Motilitätsstörung. Brown hat aus diesem Grund 1973 zwischen den „true“ und „simulated“ Fällen unterschieden. Eine Differenzierung kann daher nur über andere Befunde erfolgen, zu denen es aufgrund der verschiedenen ätiologischen Faktoren bei diesen beiden Gruppen kommt.Die Ursache ist eine erworbene Restriktion der Passage der Obliquus-superior-Sehne durch die Trochlea. Hierzu kommt es überwiegend durch eine/n palpierbare/n Anschwellung/Knoten der Sehne hinter der Trochlea. Dies kann auf drei Arten entstehen: 1. kurz nach der Geburt, aufgrund von bei Geburt noch nicht voll entwickelter Gleitverhältnisse, die für eine glatte Passage der Sehne durch die Trochlea erforderlich sind; 2. durch eine Entzündung im Rahmen einer systemischen Erkrankung wie z. B. Rheuma oder auch ohne Bezug zu einer bestimmten Erkrankung; 3. durch stumpfes Orbitatrauma entstandenes Hämatom. Zusätzlich kann es zu einer Verengung der Trochleapassage bei einer ausgeprägten Entzündung unter Beteiligung der Trochlea mit Schwellung und starker Druckschmerzhaftigkeit über der Trochlea kommen, was einer Tendovaginitis stenosans der Hand entspricht.Das therapeutische Vorgehen ist bei diesen vier Varianten ganz unterschiedlich und hängt von der Art der jeweiligen Ursache ab.Eine Verdickung der Obliquus-superior-Sehne hinter der Trochlea ist die Ursache für die Motilitätsstörung beim „erworbenen Jaensch-Brown-Syndrom“. Hierzu kann es über drei verschiedene Arten kommen, bei denen jeweils unterschiedliche therapeutische Maßnahmen erforderlich sind." @default.
