The historical, clinical, histologic and roentgenographic findings of primary hyperparathyroidism have been discussed in some detail and radiographs illustrating the skeletal involvement before and after treatment are presented. 2. The primary roentgenographic findings in untreated hyperparathyroidism are: (a) Nephrolithiasis or nephrocalcinosis (67 to 77 per cent of cases). (b) Diffuse resorption of the skeleton with cortical thinning and delicate, indistinct trabeculation. (c) Bending deformities and pathologic fractures of bone. (d) Metaplasia of bone with formation of cysts, brown tumors (pseudocysts) and, on occasion, true giant-cell tumors. (e) Subperiosteal resorption of bone, particularly in the hands and lamina dura of the teeth. 3. The roentgenographic findings in treated cases are: (a) Return to normal in those bones in which subperiosteal resorption has occurred; reconstruction, mineralization and fracture healing. (b) Osteoid matrix production with mineralization of the pseudocysts or brown tumors, frequently becoming more dense than the surrounding bone. (c) Failure of the true cysts to fill in and heal. (d) In healing, the cranial bones may resemble Paget's disease. (e) Healing bones never return to normal texture; the healed bones usually have a greater density than normal due to the altered matrix." @default.
