- W4293231280 abstract "Introduction and aims of the study: Diagnostic hysteroscopy enables the direct visualization of the cervical canal and the uterine cavity, allowing an accurate assessment of intrauterine alterations. As for endometritis and endometrial hyperplasia diagnosis, endometrial biopsy’s role is really crucial. Hysteroscopy and endometrial biopsy suggest valid and efficient procedures, both evaluating the endometrial factor in terms of female infertility. The purpose of this study was to estimate the reliability of hysteroscopy, routinely performed along with endometrial biopsy, by analyzing and comparing hysteroscopic and histopathological outcomes in asymptomatic infertile patients. Methods: The study included 73 infertile patients who underwent a diagnostic hysteroscopy followed by an endometrial biopsy. In patients with normal hysteroscopy findings, an evaluation of the endometrial abnormalities’ characteristics was conducted. Hysteroscopy’s sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) for endometrial alterations’ diagnosis were all calculated on the basis of pathologic reports. Results and Discussion: The hysteroscopic analysis indicated 49,3% of patients with a normal uterine cavity, 37% with endometrial polyps, 9,6% with endometrial hyperemia and 4,1% with other endometrial abnormalities (uterine adhesions, uterine septa, myomas, endometrial irregularities, endometrial atrophy), supporting hysteroscopy’s routine use in the diagnosis of uterine cavity’s functional diseases with regard to infertility’s investigation. Among patients with a normal uterine cavity at hysteroscopic examination (36 women), 63,9% had a normal biopsy outcome too. However, at least one histopathological abnormal aspect was diagnosed at a ratio of 36,1%, pointing out a remarkable prevalence of alterations when endometrial biopsy is performed, even with normal hysteroscopic findings. Conclusions: Hysteroscopy demonstrated intrauterine alterations in half of the infertile women. Diagnostic hysteroscopy and concurrent endometrial biopsy should be used as complementary diagnostic and therapeutic approaches, especially in cases of infertile patients." @default.
