- W4295258532 abstract "A 52-year-old man with T3aN1M0 invasive bladder cancer with bony metastasis, currently being treated with enfortumab-vedotin, presented to dermatology for a full body eruption that began 3 months after initiating therapy. He initially improved with oral steroids but recurred after his subsequent treatment cycles. On examination, he had discrete, monomorphous erythematous 1 to 2 mm papules distributed across the proximal extremities. Two punch biopsies were performed showing intraepidermal vesicular dermatitis with epidermal dysmaturation. The findings were attributed to enfortumab-vedotin. He was started on clobetasol ointment with improvement and his infusions were continued. Few cases of enfortumab-induced eruptions have been reported. Biopsies from other case reports share a common histopathologic feature of epidermal dysmaturation, but there appears to be a variable inflammatory reponse. Intraepidermal vesiculation, present in our patient, is a novel finding along with epidermal dysmaturation which appears to be a unifying feature of rashes secondary to enfortumab. With the increasing use of enfortumab, it is important for clinicians to be aware of the various adverse events related to this therapy. This case demonstrates novel histopathologic findings with a favorable treatment response that allowed the patient to continue enfortumab therapy uninterrupted." @default.
