Exaktes axilläres Staging ist daher für ein erfolgreiches klinisches Management von Patientinnen mit dieser Erkrankung von größter Wichtigkeit. Methodik: Im Rahmen einer Literaturübersicht wird die axilläre Lymphknotendissektion als derzeitiger „Gold-Standard“ des axillären Stagings präsentiert sowie im Lichte möglicher Alternativverfahren betrachtet. Ergebnisse: Die axilläre Lymphknotendissektion trägt wesentlich zur Verwirklichung folgender, für eine effiziente Behandlung des invasiven Mammakarzinoms unabdingbarer, Grundvoraussetzungen bei: 1. exakte Stadieneinteilung der Erkrankung, 2. zuverlässige Feststellung der individuellen Prognose, 3. Möglichkeit zur individuell risikoadaptierten adjuvanten Therapieplanung sowie 4. wirksame lokale Tumorkontrolle in der Axilla. Bei technischer Beherrschung sowie exakter Durchführung der Methode sind postoperative Komplikationen selten. Alternative axilläre Stagingverfahren (Sentinel-Lymphknoten-Diagnostik, PET-Scan etc.) sind zum gegenwärtigen Zeitpunkt noch nicht ausreichend standardisiert, um die axilläre Lymphknotendissektion in der klinischen Routine ersetzen zu können. Schlußfolgerungen: Die axilläre Lymphknotendissektion bleibt nach wie vor unverzichtbares Standardverfahren zur Evaluierung des axillären Lymphknotenstatus beim invasiven Mammakarzinom und wird auch in Zukunft nicht vollständig durch alternative Stagingverfahren ersetzt werden können." @default.
