- W4300108546 abstract "During the last decade there have been some important developments in understanding the social determinants of the distribution of health and illness (Bartley et al, 1998). There has been a shift from presuming that the main link between poverty and ill health is to do with the material conditions of people’s lives, to the idea that psychosocial mechanisms may be just as important. This suggestion originally arose from national-level studies that found that increases in wealth did not necessarily lead to improvements in health. In particular, Wilkinson (1996) has shown that the distribution of income and wealth within a nation can influence life expectancy. He suggests that after a certain point of economic development has been reached – what is sometimes termed the ‘epidemiological transition’ – it is relative rather than absolute poverty that is critical to overall health status. This means that health status must be associated with things other than just absolute material deprivation. Wilkinson suggests that the more unequal the distribution of resources in a society the more likely it is that the social organisation will have health-damaging consequences for individuals. This means that health outcomes must, in some way, be linked to the experience of relative poverty. Attempts to make sense of this have led to the formation of a research agenda which concentrates on “the psychological stress, relative deprivation and psychosocial injuries of inequality structures” (Elstad, 1998, p 40). This agenda has produced growing evidence that variations in certain aspects of psychosocial life – such as sense of control, perceived social status, the strength of social affiliations, self-esteem, feelings of ontological insecurity and so on – lead to variations in health outcomes (Elstad, 1998;Wilkinson, 1996;Wilkinson et al, 1998)." @default.
