- W4306177524 abstract "Introduction: This trial assessed whether pelvic intraoperative neuromonitoring (pIONM) could improve functional outcomes in patients that underwent total mesorectal excision (TME) for rectal cancer. Methods: It was a 2-arm, randomized, controlled, multicenter clinical trial that included 189 patients with rectal cancer who underwent TMEs in 8 centers. TMEs were performed with (pIONM, n = 90) or without pIONM (control, n = 99). The groups were stratified according to neoadjuvant chemoradiotherapy and sex. Data were analyzed according to a modified intention-to-treat (mITT) protocol. A secondary endpoint of the study was anorectal functional outcomes and this is presented here. Results: The final analysis included 171 patients with mITT. pIONM was associated with better ano-(neo-)rectal functions. During the 12-month observation period, postoperative fecal incontinence was significantly worse in the control group than in the pIONM group. This was indicated by the smaller absolute change in Wexner score, compared with baseline, in the pIONM group (3.6 ± 5.2) compared with the control group (5.5 ± 5.3, p = 0.044). Incontinence pads were required in 46% of patients in the pIONM group and 63% in the control group (p = 0.049). The feeling of fragmented emptying was reported by significantly fewer patients in the pIONM group (56%) than in the control group (75%). Conclusion: pIONM improved ano-(neo-)rectal functional outcomes in patients with rectal cancer that underwent TME." @default.
