- W4306177761 abstract "Introduction: Several studies demonstrated feasibility and safety of robotic pancreatoduodenectomy (R-PD) reporting a learning curve for R-PD performed by a single surgeon that ranges from 20 to 40 cases, and a rate of conversion to open surgery that ranges from 1.1% to 5.1%. We report a case series of first 40 R-PD performed by a single surgeon previously experienced in pancreatic surgery and in robotic surgery for other indications. Methods: Between May 2018 and December 2021 40 consecutive R-PD were performed by the same operator with da Vinci Xi. Intraoperative and perioperative outcomes were evaluated. Borderline resectable or locally advanced tumors were considered the only surgical exclusion criteria for the robotic approach. Results: All the procedures were performed with a full-robotic technique, both for resective and reconstructive phases. No conversion to open surgery was reported. Sixteen of 40 patients (40%) had a BMI greater than or equal to 25 kg/m2, while 18 of 40 patients (45%) had undergone previous abdominal surgery. Mean console time was 282,7 ± 39,5 minutes and mean operative time was 434,9 ± 112,1 minutes. Five patients (12,5%) had major complication, only 2 (5%) had grade B postoperative pancreatic fistula. The median postoperative stay was 10 days, and no 30-day mortality was registered. Conclusion: Extensive previous experience in both pancreatic and robotic surgery together with the availability of the da Vinci Xi platform allows to perform challenging procedures such as R-PD with low risk of conversion to open surgery, regardless of the learning curve specific for this procedure." @default.
