- W4306178766 abstract "INTRODUCTION: Many patients continue to experience significant coughing after antireflux surgery (ARS), which may compromise repair integrity. Magnetic sphincter augmentation (MSA) has emerged as an alternative to fundoplication that effectively augments the lower esophageal sphincter without need for gastric anatomy alteration. We aim to study whether MSA may be more protective compared with traditional fundoplication in the setting of chronic cough. METHODS: A retrospective review of a prospectively maintained database identified patients who underwent ARS. Primary outcome was radiographic recurrence or revisional operation rate. Secondary outcomes included rates of postoperative GERD recurrence, endoscopic intervention, and antireflux medication use. RESULTS: A total of 283 patients underwent ARS: 248 laparoscopic fundoplication and 35 MSA placement. Of these, 42% were male, mean age was 57 years, and mean BMI 26.3 kg/m2. Average follow-up was 358 days. 64 (22%) had radiographic recurrence, and 14 (5%) underwent revisional operation. Compared with the MSA cough group, the Nissen fundoplication cough group demonstrated more GERD recurrence (42% vs 11%, p = 0.01) and trended toward increased rate of revisional operation (16% vs 0%, p = 0.09). Overall, cough patients had worse outcomes compared with noncough, including higher radiographic recurrence (38% vs 12%, p = 0.001), GERD recurrence (36% vs 18%, p = 0.001), reflux medication use (67% vs 52%, p = 0.016), and postoperative endoscopic intervention rate (28% vs 16%, p = 0.028). CONCLUSION: MSA should be the antireflux procedure of choice for GERD patients with chronic cough due to lower GERD recurrence and revisional operation rate." @default.
