- W4306179212 abstract "INTRODUCTION: Peroral endoscopic myotomy (POEM) is an effective alternative to Heller myotomy with partial fundoplication, yet GERD remains the main complaint after this procedure. This study aimed to determine the incidence of GERD after POEM and to identify its predisposing factors. METHODS: Records of patients who underwent POEM for achalasia, esophagogastric junction outlet obstruction, or jackhammer esophagus through 8 years at our institution were reviewed. Postoperative objective GERD was defined as LA grade C or D esophagitis or DeMeester score >14.7. Preoperative clinical, endoscopic, physiologic, and surgical technique data were analyzed for impact on post-POEM GERD. RESULTS: The study population consisted of 183 patients. At a mean follow-up of 21.7 months, 93.4% had a favorable outcome (Eckardt score <3) and 38.9% complained of GERD (heartburn score >3 on the GERD-Health-Related Quality of Life questionnaire). Ninety-eight patients completed postoperative objective testing, of whom 49% had objective GERD. Women were more likely to complain of GERD (48.3 vs 29.5, p = 0.01), but the rate of objective GERD was similar between sexes (45.6 vs 52.7, p = 0.54). Demographics, procedure indications, duration of symptoms and longer length of myotomy or cardia extension had no impact on the rate of post-POEM GERD. Patients with GERD were found to have a lower esophageal sphincter (LES) pressure (41.1 vs 49.2 mmHg, p = 0.047). CONCLUSION: Objective GERD after POEM is more common than symptomatic reports. Procedure indication or myotomy length has no impact on post-POEM GERD. Patients with lower preoperative LES pressure are at higher risk for post-POEM GERD and may be better served by myotomy with partial fundoplication." @default.
