- W4308463091 abstract "距骨下関節脱臼は交通外傷や高所転落などの高エネルギー外傷により生じ，全脱臼の1-2％と比較的稀であるため，治療方針に関して確立した指針は得られていない．今回，足部の多発骨折を合併した距骨下関節脱臼の1例を経験したので文献的考察を交えて報告する．症例は45歳男性．建設現場の足場より転落し受傷．近医を受診し，精査加療目的に当科紹介となった．CTで距骨下関節脱臼・距骨後突起骨折・舟状骨骨折・踵骨前方突起骨折・右腓骨遠位裂離骨折・第5中足骨骨折を認めた．全身麻酔下に距骨下関節脱臼に対する徒手整復および踵骨前方突起骨折に対する観血的整復固定術を行った．後療法として外固定はギプス・シーネによる固定を5週間継続した．荷重は術後4週間完全免荷とし，以降より部分荷重を開始し術後10週で全荷重可能となった．職場復帰も可能となり，日常生活機能としては問題ないが，今後の関節症性変化の進行に留意した長期フォローが必要である．" @default.
