- W4311593627 abstract "Introduction: More individuals with childhood chronic illness are reaching adulthood, and many continue their care at pediatric centers. Understanding this population may identify opportunities for improved care or safe transition to adult medical centers. This study aims to describe characteristics and trends of adult patients (age ≥ 19) who are admitted to Pediatric Intensive Care Units (PICUs) in the United States over a ten-year period. Methods: This study focuses on all admissions from patients ages ≥19 at PICUs that contribute data to the Virtual PICU Systems (VPS) Database from 1/1/2009 until 12/31/2019. Data obtained includes principal diagnoses, readmissions, PICU length of stay, presence of significant ongoing conditions, and mortality. The data was analyzed to assess trends in admissions and readmissions, diagnostic categories, and outcomes, with data broken down by year and by major age groupings, as well as separating out the cardiac and non-cardiac patient groups. Results: There are a total of 32,794 admissions of patients ≥19 over the study period with the majority ages 19-21 (22,004). The number of admissions increased over the years with 1591 admissions in 2009 and 3,666 admissions in 2019. The percentage of readmissions increased over the years, with 2009 having 24.6% readmissions and 2019 having 58.8% readmissions. The odds of being a cardiac patient increased with each age group (P value < 0.0001) but was not significantly different over the 10 years. There was an increase in mortality for older, non-cardiac patients with the 19-21 year group having a mortality rate of 3.1% and the ≥40 group having a 9.1% mortality rate. Cardiac patients had decreased mortality with the mortality rate in the 19-21 year age group being 4.1% and the ≥40-year age group having a mortality rate of 1% (OR 0.23, 95% CI 0.09-0.64). No trends in mortality across the 10 years. Conclusions: Adults account for a meaningful number of PICU admissions with increases in readmissions over the last 10 years. Pediatric centers may need expertise from providers skilled in caring for adult comorbidities, as older patients have an increased mortality rate. This is especially true for units that care for adults with congenital heart disease, as the cardiac patients reach greater age groups still being admitted to PICUs." @default.
