- W4312277021 abstract "<b>Introduction:</b> Chronic obstructive pulmonary disease (COPD) patients who require ambulatory supplemental oxygen usually perform oxygen titration through 6-minute walk test (6MWT). One-minute sit-to-stand (1-STS) was previously described as a potential alternative to 6MWT for oxygen desaturation but not oxygen titration. <b>Aim:</b> Evaluate concordance between 1-STS and 6MWT in titrating oxygen therapy on COPD <b>Methods:</b> Prospective study conducted at a university tertiary hospital including COPD patients under long-term oxygen therapy (LTOT). Patients performed both 1-STS and, after adequate rest, 6MWT <b>Results:</b> Twenty-six patients (59.3% male, mean 66 years) were analyzed, of whom 92.5% were current/former smokers. Overall concordance between maximum FiO<sub>2</sub> on 6MWT and 1-STS was fair (k=0.339), with 1-STS presenting equal maximum FiO<sub>2</sub> needs in 46.2% of patients and overestimating in 7.7%. Underestimation increased when maximum FiO<sub>2</sub>>0.31. In fact, concordance with 1-STS was moderate (69.2%; k=0.447) in patients with maximum FiO<sub>2</sub>≤0.31 in 6MWT. Concordance was moderate (66.7%; k=0.474) in patients without severe exacerbations in the last 5 years. There was higher concordance in those without long-term NIV (k=0.388 vs 0.222 in NIV), without OSA (k=0.341 vs 0.286 in Overlap Syndrome) and under LABA+LAMA without ICS (k=0.557 vs 0.214 in triple inhaled therapy). Those with above median DLCO had moderate maximum FiO2 concordance (64.3%, k=0.517), as in patients with both above median DLCO and FEV1 (71.4%; k=0.533). <b>Conclusions:</b> 1-STS may be a suitable alternative for portable oxygen titration until 6MWT can be performed, especially for above average clinical and functional status patients in whom a FiO<sub>2</sub>≤0.31 is expected." @default.
