- W4312281647 abstract "Introduction: Blood loss is an important factor in determining liver resection outcomes. Energy devices were invented in order to minimize blood loss in liver parenchyma resection. Thulium-Doped Fiber laser (TDFL) emitting at 1940nm never been recorded as an energy device beforehand. Method: This is a retrospective study from all the procedures in 2019-2020 in Prof. dr. R.D. Kandou General Hospital. Using TDFL 1940nm, patients with pre-existing secondary liver tumours with Child-Pugh score A were assigned to receive liver resections. Complexities of procedure, blood loss, liver transection speed, morbidities, and mortalities including PHLF were recorded and evaluated. Result: From total 23 liver resections performed, 6 (26.08%) procedures using TDFL as energy device were performed towards secondary liver tumours by a single liver surgeon, 83.3% were liver MCRC and 17.7% were GIST metastases. Liver resections performed were bisegmentectomies (33.3%), Systematic Extended Right Posterior Sectionectomies (SERPS) (16.7%), central hepatectomy (16.7%), and non-anatomical resections (33.3%). Intraoperative characteristics showed that estimated blood loss is 529 + 47 ml, with the procedure duration around 63.3 + 33.9 mins, liver transection speed of 1.69 cm2/min, and blood transfusions 1.25 + 0.7 units. No mortalities nor PHLF were recorded in 30 days post-op surveillance, but biloma (16.7%) occurred. The technical and opinions of the author towards TDFL 1940nm usage was mentioned after. Conclusion: TDFL is a promising energy devices in secondary liver tumors. Complexities of the procedures and the inability of TDFL to seal vessels and bile ducts relate to the liver transection speed." @default.
