- W4312372275 abstract "<b>Introduction:</b> Bronchiolitis obliterans syndrome (BOS) is the predominant pulmonary manifestation of Graft-versus-Host disease. Diagnosis is made using spirometry; however, this is thought to be insensitive for detection of early disease. Forced oscillatory technique (FOT) is a simple procedure, using ultrasound waves for measuring lung impedance, and identifying small airway disease. <b>Aims:</b> To investigate the utility of FOT for diagnosis of BOS and correlations between FOT parameters and spirometry/plethysmography in patients post AHCT <b>Methods:</b> We prospectively recruited patients prior to or following AHCT. All subjects performed FOT and full PFT9s. Correlation analyses between FOT parameters (R5,X5,R19,X19,R5-19,DELXrs), and PFT parameters (FEV1,FEV1/FVC,FEF25-75,RV/TLC,Raw,sGaw) were performed (Spearman9s rank). Mann-Whitney test was used to compare different FOT parameters in BOS vs non-BOS patients. <b>Results:</b> 30 patients were enrolled to date (mean age 48y, 47% females, mean time since transplant 40 months). 7 patients were diagnosed with BOS. A negative statistically significant correlation was found between FEV1 and R5 (r=-0.759, p<0.001), X5 (r=-0.778, p<0.001) and R5-R19 (r=-0.501, p=0.006), between FEV1/FVC and R5 (r=-0.462, p<0.01), X5 (r=-0.563, p=0.001) and R5-R19 (r=-0.552, p=0.002), and between FEF25-75 to R5 (r=-0.632, p<0.001) and X5 (r=-0.726, p<0.001). There were significant differences in R5, X5 and R19 between BOS and non-BOS patients (p<0.001). <b>Conclusions:</b> FOT is a useful tool for diagnosis of BOS following Allo-HCT. Future prospective longitudinal data may indicate its sensitivity over spirometry in this population." @default.
