- W4312462018 abstract "Penetrating Keratoplasty (PK) was the surgical procedure of choice for keratoconus until the late twentieth century. Newer optical methods to improve the quality of vision and the introduction of collagen cross-linking to arrest the progression of disease have vastly reduced the need for surgical interventions. When indicated, refinements in surgical techniques, like the Deep Anterior Lamellar Keratoplasty (DALK) have been changing the practice patterns among most surgeons since early twenty-first century. Although the results with both procedures are comparable, there remain situations when PK is still preferred as compared to DALK, even at the cost of increased risk of graft rejection. This chapter provides a comprehensive understanding about the relevance of penetrating keratoplasty in keratoconus in the current context, focusing on the surgical nuances, its associated complications, and its relative merits and demerits in comparison with DALK. This will help a surgeon make an informed decision regarding the surgical management for keratoconus." @default.
