- W4312538530 abstract "Introduction: Originally described in 2002, Acinar Cystic Transformation (ACT), formerly known as Acinar Cell Cystadenoma, is an extremely rare phenomenon, with only about seventy cases described in the literature. Case Report: In this case study, we describe a 30-year-old otherwise healthy female patient with Ehlers-Danlos Syndrome who intended to donate a kidney but was found to have conglomerate cystic lesions and nodular calcifications throughout the body and tail of the pancreas on cross-sectional imaging. Endoscopic ultrasound (EUS) from the growing cyst revealed atypical changes. Given the diagnostic dilemma and need to rule out malignant process prior to organ donation, a laparoscopic distal pancreatectomy and splenectomy was performed. The patient recovered well post-operatively. Final pathology revealed Acinar Cystic Transformation and zero out of seven lymph nodes positive for any malignancy. Discussion: While considered non-malignant, ACT poses a diagnostic dilemma for surgeons, radiologists, and pathologists. Most commonly, ACTs are observed in young females, as was the case with our patient. The differential is oftentimes broad and variable including serous cystic neoplasm, mucinous neoplasms, adenocarcinoma and neuroendocrine neoplasm. Fortunately, in our patient, the cysts were located in the body and tail of the pancreas which stands in stark contrast to other case reports, in which the majority of ACT affected the head of the pancreas (50% as compared to 19%). There are many widely cited reports of treatment in the literature, ranging from observation to surgical interventions such as a Whipple procedure when the head of pancreas is involved. Conclusion: Although rare, ACTs should be considered as a differential diagnosis when dealing with cystic lesions in the pancreas. Recognizing treatment options in relation to symptoms and risk is an important step in treating patients and further defining this disease process." @default.
