- W4312629168 abstract "<b>Abstract:</b> Predicting extubation success is a challenge in clinical practice. Several predictive methods are used as tools in attempts to predict the success of extubation and to minimize failures related to increased duration of mechanical ventilation, hospital stay and worse clinical outcome. The objective of this study was to verify the ability of the Cuff Leak Test (CLT) to predict the presence of laryngospasm in the neurocritical patient population. <b>Methods:</b> Retrospective observational study with data from extubated patients from March 2016 to November 2019. We evaluated the CLT result and the prevalence of laryngospasm in the first 48 hours after extubation, as well as the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and CLT accuracy. <b>Results:</b> 121 patients were included in the study, of which 58% (N=70) were female, with a mean age of 54. The proportion of patients with a positive result for laryngospasm in the CLT was 13% (N=16), however, the prevalence of was 7% (N=9). The mean value of CLT in these patients was 49mL and 260mL in patients with a negative result. Ninety nine patients (81%) were using corticosteroids in the last 24 hours before extubation. Among the cases of stridor, 89% (N=6) had previously used corticosteroids. The prevalence of stridor was higher in patients with neurovascular disease (66%). We found a sensitivity of 22%, specificity of 88%, PPV of 12%, NPV of 93% and accuracy of 83% in CLT in neurocritical patients. <b>Conclusion:</b> The CLT, as demonstrated in the literature, has low sensitivity and should not be used alone to guide decision making regarding extubation, being necessary to associate it with physical examination and other pre-extubation tests." @default.
