- W4312846571 abstract "The arthritides of childhood can be divided into those destructive of connective-tissue elements, especially cartilage, and therefore of function, and those that are transient and nondestructive. There may be many causes of inflammation, but there is probably a single final common pathway leading to joint destruction. In all the destructive arthritides, a stimulus is present within the joint space which: (1) attracts polymorphonuclear cells, (2) causes proliferation of phagocytic synovial-lining cells, and (3) provides material that is phagocytosed. By unknown mechanisms, probably cell death, enzymes capable of destroying cartilage are released from the pathologically increased numbers of phagolysosomes. These degrade proteins, and the resulting polypeptides are themselves chemotactic and inflammatory stimuli. The whole process becomes viciously autocatalytic. The variation between specific diseases probably depends on differences between the original stimuli. In septic arthritis, the stimulus is provided by the invading bacteria. In rheumatoid arthritis and systemic lupus erythematosus, the stimuli are probably antigen-antibody-complement complexes, although infection may play an initiating role. In virus-associated arthritis, the stimulus may be either direct infection or an antigen-antibody complex in which the virus is the antigen. There is no current information about the disease-specific stimuli in the arthritides of acute rheumatic fever or sarcoidosis. Treatment can be aimed at both the specific and shared mechanisms of joint destruction. Current therapy, largely limited to anti-inflammatory agents, probably has its greatest effect on the release and activity of lysosomal enzymes. Specific antibiotics and immunolytic drugs affect disease-specific mechanisms. The latter have limited use for the treatment of arthritis per se and should be applied only to situations involving severe systemic disease." @default.
