• W4312974048 abstract "이 글은 한국 농촌사회의 보건과 의료 여건의 변화 과정을 이해하기 위한 사례 연구의 하나로, 전라북도 임실을 대상으로 살펴보고자 한다. 선행 연구, 관련 통계와 신문 기사 등과 함께 임실군 주민에 의해 작성된 『삼계일기』와 『창평일기』를 적극 활용할 것이다. 일기의 필자가 남성이기 때문에 가족 계획 등 충분히 서술되지 않은 부분이 많지만, 당시 사회상을 가공하지 않고 보여주기 때문에 일기는 흥미로운 사료로 활용될 수 있다. 일제강점기 대다수 조선인 의사들이 개업의 길로 나서면서 한국 의료는 기본적으로 시장경쟁 체제에 기반해서 작동했다. 이를 보완하기 위해 1950년대부터 정부는 각 지방에 보건소를 세워 예방의학과 기본적인 치료의학을 제공하려는 계획을 수립했지만 취약한 재정으로 인해 구상대로 실현하지 못했다. 면사무소에서 이루어진 『삼계일기』의 종두 접종 기록은 미흡한 환경 속에서 진행되었던 공중보건을 위한 정부의 노력을 보여준다. 1960년대 들어 임실을 비롯해 군 단위마다 보건소가 설치되었지만, 필요한 인력을 구하지 못해 제기능을 발휘하지 못하는 경우가 많았다. 의료기관이 근처에 없었기 때문에 농촌에서는 제때 치료를 받지 못하고 병을 키우는 경우가 드물지 않았다. 압축적 근대화 속에서 교통망이 확대되고, 농촌의료기관이 조금씩 늘어나면서 1970년대 후반부터 농촌의 의료환경이 개선되기 시작했다. 특히 1980년대 들어 특별법 제정과 전국민의료보험 제도의 출범으로 병원의 문턱이 낮아졌으며, 1990년대에 대체로 현재와 유사한 농촌지역 의료공급체제가 구축되었다. 1980년대 이후 부쩍 늘어난 『창평일기』의 질병 관련 기록은 당시 임실 주민들의 개선된 의료생활을 잘 보여준다." @default.
