- W4313015251 abstract "<b>Introduction:</b> Children with Down syndrome (DS) have a high prevalence of obstructive sleep apnoea syndrome (OSAS). Continuous positive airway pressure (CPAP) is often considered as first or second line therapy in these patients. Treatment success can be hampered by poor compliance. <b>Aims and objectives:</b> To assess CPAP adherence in children with DS and OSAS and to define predictive factors for treatment compliance. <b>Methods:</b> A retrospective chart review was performed of a cohort of children with DS. Diagnosis of OSAS was made by polysomnography and the indication for CPAP was established by a multidisciplinary team. The primary outcome measures are treatment success and duration. This is defined as use of CPAP for at least 4 hours/night during at least 3 months. Family composition, age at treatment onset, place of treatment initiation (at home or in hospital), need for multiple treatment trials are analysed as predictors of treatment outcome. <b>Results:</b> Overall therapeutic success in our study population is 79.5% (31/39). The children wore their CPAP mask for a mean of 7 hours/night (5.48–8.11). Children born in a core family have a higher chance of treatment success (p=0.011). A positive correlation was found between age at onset of CPAP and treatment success (rs= 0.41, p=0.01), with higher success rates in older children. Multiple attempts to initiate treatment were also correlated with more successful treatment (rs=0.366, p = 0.022). <b>Conclusions:</b> We found a high success rate of CPAP therapy in children with OSAS and DS. Older age (6 years), multiple trials of treatment initiation, and growing up in a core family were associated with a higher success rate." @default.
